1. Ownership & Structure

Platinum IV Therapy PLLC is an Arizona-licensed professional limited-liability company owned by healthcare professionals.

2. Medical Oversight

All IV protocols are authored and reviewed by a board-certified physician licensed in Arizona. Treatments are administered by Arizona-licensed Registered Nurses, Nurse Practitioners, or Paramedics who maintain:

• Active, unencumbered state licensure
• Basic Life Support (BLS) certification (or higher)
• Professional-liability insurance

A supervising physician remains on-call during mobile visits and on-site during clinic hours.

3. Scope of Practice

We provide elective IV hydration, vitamin injections, and related wellness services. These therapies are adjunctive and do not replace the care of your primary physician or specialist.

4. Informed Consent & Patient Rights

• Every client reviews and signs an electronic Informed Consent before treatment.
• You may refuse or withdraw consent at any time and still seek future care when appropriate.
• You have the right to ask questions and receive clear answers about risks, benefits, and alternatives.

5. Fees, Billing & Insurance

• Platinum IV Therapy is a cash-pay practice; we do not bill insurance.
• Payment is due at or before the time of service.
• Mobile visits include a concierge fee disclosed during booking.
• A superbill for potential out-of-network reimbursement is available upon request.
  

6. Professional Complaints

Concerns about clinical care or licensure compliance may be directed to the Arizona Medical Board (physicians) or the Arizona State Board of Nursing (nurses). You may also contact our Compliance Officer at (480) 678-5575; we investigate all complaints promptly and without retaliation.
